Manisha Koirala was born on 16 August 1970 into the politically prominent Koirala family in Kathmandu, Nepal. Her father, Prakash Koirala, is a politician and former Cabinet minister; her mother, Sushma Koirala, is a homemaker. She has one brother, Siddharth Koirala, who is also a Bollywood actor. Her grandfather, Bishweshwar Prasad Koirala, was prime minister of Nepal, as were two of her great-uncles, Girija Prasad Koirala and Matrika Prasad Koirala. She spent her early life in India, living in Varanasi, Delhi, and Mumbai, and attended Vasant Kanya Mahavidyalaya and Army Public School in New Delhi. Aspiring to become a doctor, she later moved to Mumbai to pursue acting.

Koirala made her acting debut with the Nepali film Pheri Bhetaula (1989) and her Hindi debut with Saudagar (1991). After a brief setback, she established herself with commercial successes in both Bollywood and Kollywood, including Bombay (1995), Agni Sakshi (1996), Indian (1996), Gupt: The Hidden Truth (1997), Kachche Dhaage (1999), Mudhalvan (1999), and Company (2002). She received critical recognition for roles in 1942: A Love Story (1994), Akele Hum Akele Tum (1995), Khamoshi: The Musical (1996), Dil Se.. (1998), and Lajja (2001). In 2012, she took a break after being diagnosed with last stage ovarian cancer, underwent a year-long treatment, and made a successful recovery by mid-2014. She returned with Dear Maya (2017) and later portrayed actress Nargis in Sanju (2018). She also starred in the Netflix production Lust Stories (2018) and the period drama series Heeramandi: The Diamond Bazaar (2024).

In addition to acting, Koirala was appointed Goodwill Ambassador for the United Nations Population Fund in 1999 for India and 2015 for Nepal, and was involved in relief works after the April 2015 Nepal earthquake. She authored the novel Healed: How Cancer Gave Me a New Life, an account of her struggle with ovarian cancer.
